Recommended Referral Process for Hearing Aid Claims

  1. Employer/TPA faxes, e-mails or mails Hearing Services Authorization Form and IME or current test results to:

  2. HearPO makes contact with the claimant to select a clinic and set up a referral within 24 hours of receipt.

  3. Once claimant is available and selects clinic, referral paperwork is faxed or mailed to the provider's office the same day. Claimant is to contact office to schedule an appointment.
  4. Provider performs hearing evaluation and hearing aid evaluation to establish baseline and makes appropriate hearing aid recommendation.
  5. Provider submits copy of hearing evaluation and "Hearing Aid Justification Questionnaire" to HearPO for Audiologic Review.
  6. HearPO's staff audiologist works with provider to create a recommendation that meets medical necessity of the claimant's hearing needs. This recommendation is forwarded to the Employer/TPA within 72 hours from receipt from the provider.
  7. Once employer/TPA receives recommendation, authorization is then sent via e-mail to HearPO at Carrie.Browning@amplifon.com.
  8. HearPO processes authorization and faxes a final authorization to the clinic within 24 hours of approval.
  9. Provider orders aids, fits patient, and submits signed final authorization form documenting that the provider and patient both agree that aids have been provided as authorized.
  10. HearPO bills employer/TPA for services/products supplied.

After this original hearing aid fitting, HearPO will only contact the employer/TPA when:

  • A repair or service request is sent to HearPO, which needs approval by the adjuster.
  • To verify an individual has a claim.
